Primary Care Network (PCN) is the name given to the organisation of GP practices who are working together to provide better, more co-ordinated, effective and sustainable care. By working together and sharing resources we are employing more skilled staff than ever before so that we give people the right care at the right time.

Together, we are responsible for the care of over 70,000 people in Northwich and the surrounding areas, working to create a better health future. The PCN consists of all the teams who work in these surgeries; we also have some staff who work across the whole of the PCN.
